Age UK would like to know your views about their Integrated Care Programme (ICP)

The Age UK ICP service brings together voluntary, health and social care services to provide personalised support to people aged 50+ who are living with long term conditions and are at risk of unplanned hospital admissions.

The service coordinates the work of organisations to provide non-medical support for people based on goals they identify as being important to them.  A key aim of the ICP is to increase independence and reduce social isolation.

Age UK Personal Independence Coordinators support delivery of the programme and are members of the Integrated Neighbourhood Teams. Support includes things like help with benefits, sorting out problems with medicines, organising equipment or help in the home, and arranging opportunities to socialise more.

We would be grateful to hear your views if you have used this service, as a carer or as a patient. We are asking three simple questions:
